How did the idea of an eyelid lift come about?
According to statistics, the plastic surgery in question is the most popular all over the world (compared to
other
types of aesthetic operations). The first mention of such a procedure appeared in the 10th century in the
writings
of Avicenna. A well-known physician believed that removing excess skin around the eyelids improves vision. At
the
same time, an analogue of modern local anesthesia was used. Also, information about blepharoplasty operations is
found in books from India.
The term used in the modern world to define this type of plastic appeared much later – in the 19th century. It
was
introduced into general use by the German ophthalmologist Karl-Ferdinand von Grefe. At first, only the removal
of
excess skin was carried out, but later they also began to remove excess fat from the area around the eyes.
Varieties of blepharoplasty
Eyelid lift surgery has several varieties. Anchor or upper blepharoplasty helps to tighten the upper eyelid.
This
removes excess skin, existing fat deposits, and in some cases, also muscles. In the case of the so-called Asian
blepharoplasty or Singapore, on the contrary, a fold is formed, which is absent in the Asian race.
Sublicial or inferior blepharoplasty is performed on the lower eyelid. It is prescribed for more serious changes
and
is carried out after making an incision under the eyelash growth line. Such surgical intervention allows you to
redistribute or remove fat, eliminate sagging skin and tighten the drooping corner of the eye. If necessary,
such
plastic surgery can be performed transconjunctively, that is, without a long incision. In this case, the
dissection
is performed from the inside of the eyelid, and there are no stitches at all after the end of the operation.
With
volumetric or fat-saving blepharoplasty, excess fat is not removed from the eye area – it is simply
redistributed.
Circular blepharoplasty affects both upper and lower eyelids. If a combined operation is performed, it may
include,
for example, plastic surgery of the upper eyelid, performed according to the classical technique, and plastic
surgery of the lower eyelid, performed transconjunctively.
The history of laser blepharoplasty
The laser type of blepharoplasty, which is practiced in modern plastic surgery clinics, differs from the
classical
instrument: in this case, a laser knife is used. This provides a significant simplification of the
rehabilitation
process.
The first time a blepharoplasty operation using a laser was performed in 1980 by Baker. After four years of
practice, he published the results of forty operations. The conclusion was positive, because this type of
correction
of the shape of the eyelids fully corresponded to the expectations of patients. To make sure of this, you can
consider the section blepharoplasty of the eyelids: blepharoplasty before and after.
Interest in such a eyelid lift intensified in the nineties of the twentieth century, when they began to use a
high-energy CO2 laser, which is called UltraPulse. It emits a pulsed high-frequency beam capable of making neat
incisions with the minimum possible area of thermal damage. At the same time, the frequency of the pulsed laser
and
its energy can be adjusted until an ideal combination is achieved for performing a specific operation.
It is worth noting that the radiation frequency of the device in question ranges from 1000-3000 Hertz. As for
the
diameter of the laser beam, it was only 0.2 mm. At the same time, the area of thermal damage is 115 microns.
About the features of laser technologies
It was found that in most cases it is more effective to perform plastic surgery on the eyelids using laser
technologies, rather than using classical steel tools. There are several reasons for this:
-
the tip with a laser performs a triple function: it works as a cutting tool, a cauterizer for cauterizing
tissues
and a device for blunt dissection, autopsy;
- the operation execution time is reduced;
-
laser dissection allows for highly professional operations on the thinnest vascular structures (the muscle
that
lifts the upper eyelid, as well as the Muller muscle and tarsal);
- low risk of tissue distortion or hematoma formation is ensured;
-
during the surgical intervention, almost complete hemostasis is observed (the body`s system that ensures the
preservation of the liquid state of the blood in its normal state and stops bleeding in case of injury);
- the eyelid surgery recovery time and return to normal life is significantly reduced;
- intraoperative transfer of instruments is reduced.
Surgical blepharoplasty versus laser
An overhanging upper eyelid or an excess of fat deposits in the lower one is a serious aesthetic problem that
needs
to be solved. These changes may be the result of genetic predisposition or age-related changes. In any case,
blepharoplasty helps to rejuvenate the face, gives the look youth and freshness. How to decide which of the two
types of this minimally invasive surgical intervention should be resorted to? Let`s consider the features of
these
techniques.
Unlike surgical or classical laser blepharoplasty is performed by evaporation of excess tissue by exposure to
laser
directed radiation. This non-surgical technique helps not only to eliminate the problem of the overhanging
eyelid,
but also smoothes the nasolacrimal furrow and wrinkles in the corners of the eyes. The peculiarity and advantage
of
using the laser method is the penetration of the beam by 1 micrometer into the skin. At the same time, the upper
layer of the skin is not injured, as it happens when performing an incision with a surgical scalpel.
Laser blepharoplasty, the price of which is mainly lower in the Turkish clinic than in other countries, is being
performed more and more often. Considering the above, the following advantages of this type of plastic surgery
can
be distinguished:
-
a reduced number of contraindications (unlike a classical or surgical procedure, laser has much fewer
contraindications, therefore it can be prescribed to a larger number of patients who have sought help);
-
minimal injury (the laser seals the blood vessels in the affected area, so the patient receives minimal
injuries
during blepharoplasty, and the risk of bleeding tends to zero);
- effectiveness (in most cases, one procedure is enough to get the desired effect);
-
convenience (due to the fact that when exposed to a transparent laser beam, the doctor does not cover part
of the
treated area with a scalpel, the result of the operation will be better and more accurate);
- the speed of execution (the laser exposure itself takes from 15 to 30 minutes, and rehabilitation takes 5-7
days).
It is noticed that from surgical intervention, that is, from classical blepharoplasty, wide incisions left by a
scalpel remain. At the same time, there is still a risk of damage to the eyeball if the surgeon`s hand trembles.
It
should also be taken into account that wounds from conventional surgery heal longer than from laser
intervention.
Among the disadvantages of laser plastics, laser blepharoplasty cost can be distinguished (compared to the usual
classical).
