Dental Implants

A dental implant is a small titanium or zirconia post placed in the jawbone to act as an artificial tooth root. Once the bone has bonded to it, it supports a crown, a bridge or a full arch of fixed teeth. Implants are one of several options for replacing missing teeth, alongside bridges on natural teeth and removable dentures. They are also a process rather than a single appointment. Understanding the stages, and the healing time between them, helps you plan a trip realistically.

Who is a good candidate#

Implants are usually considered for adults who have lost one or more teeth, or whose remaining teeth cannot be saved. The dentist looks at three things in particular: whether there is enough healthy jawbone, whether the gums are healthy, and whether your general health supports healing.

Implants may need to wait, or may not be the right option, if you:

• have active gum disease or untreated decay, which is treated first;
• smoke heavily and cannot stop around the time of treatment, as smoking affects healing;
• have uncontrolled diabetes or another condition that affects healing;
• take, or have taken, certain medicines that affect the bone, such as some treatments for osteoporosis or cancer;
• have had radiotherapy to the head and neck;
• are still growing, as implants are generally placed only once the jaw has finished developing;
• are pregnant.

Many of these do not rule implants out completely but change the plan. Be open about your medical history and medications so the dentist can advise you properly.

Techniques and options#

Single implants and bridges#

A single implant with a crown replaces one missing tooth without touching the teeth on either side. Where several teeth are missing in a row, two or more implants can support a bridge, so you do not need one implant per tooth.

Full-arch treatment and All-on-4#

For people missing most or all teeth in a jaw, or whose remaining teeth are failing, a full arch of fixed teeth can be supported on a small number of implants. All-on-4, which uses four implants per jaw, is one of the options offered. In suitable cases, a fixed temporary arch can be fitted within a few days of surgery, with the final teeth made after healing. Some patients need more implants, depending on bone and bite.

Bone grafting and sinus lift#

If the bone is too thin or too low, it may need building up first. This can be done with a bone graft or, in the upper back jaw, a sinus lift. Depending on the amount of grafting, it may be done at the same time as the implants or months beforehand, which can add a trip.

How treatment is carried out#

Implant treatment usually follows these stages:

1. Assessment and planning. An examination, X-rays and usually a 3D scan (CBCT) show the bone and nerve positions. The dentist confirms the plan and number of implants.
2. Preparation. Teeth that cannot be saved are removed, and any gum disease is treated. Some teeth may need a root canal treatment instead of removal.
3. Implant placement. Under local anaesthesia, the implant is placed into the bone. Sedation can be discussed if you are anxious. Placing one implant usually takes around an hour; full-arch cases take longer.
4. Healing (osseointegration). The bone bonds to the implant over a period that is typically around three to six months, depending on the site and bone quality. A temporary tooth or arch is usually fitted in the meantime.
5. Final restoration. An abutment is connected, impressions or digital scans are taken, and the final crown, bridge or arch is fitted and adjusted.

Because of the healing stage, most implant cases involve two trips: the first for placement, the second a few months later for the final teeth. Cases that need bone grafting first may need more than two trips. The dentist decides whether earlier loading is possible in your case.

Recovery and results#

• First days: swelling, bruising and soreness are common, usually managed with prescribed pain relief and cold compresses. A soft diet is advised.
• First weeks: stitches are removed or dissolve, and gums heal. You avoid chewing hard foods on the implants.
• Healing months: you wear a temporary solution while the bone integrates.
• After the final teeth: you chew normally, keeping to the cleaning routine you were shown.

Risks and limitations#

• Swelling, bruising, pain and bleeding after surgery.
• Infection around the implant.
• An implant that does not integrate with the bone and needs replacing.
• Nerve irritation in the lower jaw, which can cause numbness of the lip or chin, usually temporary.
• Sinus problems after upper-jaw implants or sinus lifts.
• Inflammation and bone loss around implants over the years (peri-implantitis), linked to poor cleaning and smoking.
• Wear or chipping of the crown or bridge, which may need repair or replacement.

Planning your treatment in Turkey#

Planning starts with a recent panoramic X-ray or CBCT scan if you have one, photos of your teeth and your medical history. The case is reviewed, and you receive a written treatment plan and quote, including the number of implants, the stages and the expected number of trips.

The first trip is often around five to seven days, allowing time for the scan, any extractions, implant placement and a check before you fly.
