The Art of the Brazilian Butt Lift: How Turkish Surgeons Achieve Natural-Looking Results

A natural-looking BBL is one where the buttocks, hips and waist look in proportion to your frame, with smooth transitions and no obvious edges, rather than one defined by size. In a Brazilian butt lift, fat is removed by liposuction from areas such as the abdomen, flanks and lower back and part of it is transferred to the buttocks, so the natural look depends as much on how the waist and back are contoured as on the volume added. Your frame, your available fat, your skin and the volume that can safely be transferred all set the limits.

This article explains how surgeons plan the shape, which BBL shapes patients commonly ask for and why some results look unnatural. For candidacy, safety, recovery and planning, see the BBL overview. Plastic Surgery Turkey Group (PSTG) coordinates the trip; the treating surgeon at a contracted healthcare institution decides the technique and the volume.

What a natural BBL looks like#

A natural result is about shape, transition and balance. The aim is to enhance the buttocks in a way that fits the body you already have. Surgeons usually think about five elements:

  • Waist-to-hip ratio. Liposuction of the waist, abdomen and lower back creates the narrower waist that makes the buttocks look fuller. Much of the change people notice comes from this step.
  • Smooth hip contours. Small amounts of fat in the hollow at the side of the hips, where planned, soften the outline without making the figure look wider.
  • A rounded, lifted shape. Fullness at the top of the buttock gives a lifted look, while the lower buttock is filled more gently so the result does not look heavy.
  • Projection that suits your frame. The amount of projection is matched to your height, pelvis and waist, so the buttocks look like they belong to your body.
  • Natural transitions. The lower back, hips, buttocks and thighs blend into each other without ledges, steps or obvious edges.

In everyday life, a natural BBL tends to read as a more balanced figure rather than as surgery: clothes fit differently, and the change usually looks in keeping with the rest of the body.

Why some BBL results look unnatural#

Most unnatural results come from asking the body to carry more change than it can, or from uneven contouring. Common reasons include:

  • Too much volume. More fat than the frame and skin can carry looks out of proportion and stretches the skin.
  • Overfilled hips. Too much fat at the sides of the hips widens the figure instead of shaping it.
  • Abrupt transitions. A visible ledge between the lower back and the buttocks draws attention to the change.
  • Uneven liposuction. Over-treated donor areas can be left with dents or ridges.
  • Asymmetry. Fat does not always survive equally on both sides.

Some of these can be softened later, sometimes with a second session, but it is easier to plan a proportionate result from the start.

Common BBL shapes#

Patients often describe what they want using a few familiar shapes. The table shows what each aims for and its limits. Your surgeon will tell you which is realistic for your body.

ShapeWhat it aims forCan suitLimits
Soft hourglassFuller hips and buttocks with a narrower waistPatients with enough donor fat and some natural waist definitionNeeds enough fat for both hips and buttocks
Athletic curveModerate volume, a lifted look at the top of the buttock, subtle projectionSome lean or active patients, and some menThe change is modest
Hip dip correctionFilling the hollow at the side of the hipsPatients bothered mainly by the side profileHip dips partly reflect bone structure, so they can be softened rather than erased
Slim-thickA narrow waist with more projected buttocksPatients carrying fat on the abdomen and flanksRelies on waist contouring as much as on volume
High projectionA larger, more visible changePatients with plenty of donor fat and good skinLimited by the safe volume per session and skin tolerance; can look less natural

Men usually aim for a different outline, with projection but slim hips; our article on BBL for men explains how the plan changes.

How technique affects the look#

Technique matters for the look mainly in two places: how evenly the donor areas are contoured, and how the fat is distributed between the upper buttock, the central buttock, the sides and any hip dips. Careful liposuction avoids dents in the donor areas, and building the shape gradually in small amounts helps the transitions look smooth. The exact technique is the treating surgeon's choice; how the operation is carried out is described on the main BBL page.

Safety and a natural result go together#

The limits that make a BBL safer also tend to keep it natural. Keeping the volume within what your tissue can accept avoids an overfilled look. Surgeons place fat in the layer under the skin, above the muscle, which lowers the risk of fat embolism, the most serious complication associated with a BBL, but does not remove it. Other risks include blood clots, fluid collections, infection, fat necrosis, irregular contours, asymmetry and more volume loss than expected.

It is reasonable to ask at consultation where the fat will be placed, how the surgeon confirms it and how many BBLs they perform.

What limits how natural your result can be#

  • Donor fat. You can only transfer the fat you have, and some of it is not usable.
  • Your frame. The width of your pelvis and ribcage and the shape of your hip bones set the proportions.
  • Skin quality. Fat transfer adds volume but does not tighten loose skin. Marked sagging may need a different or additional operation.
  • Fat survival. Part of the transferred fat is reabsorbed, the amount varies, and the shape is judged after around three to six months.
  • Safe volume per session. Very large changes are not done in one session. A second session is sometimes discussed later.
  • Weight after surgery. Fat that survives behaves like other body fat, so gaining or losing a lot of weight changes the result.

Our guide to whether a BBL suits your body goes through these factors as candidacy requirements.

Aftercare and the final shape#

How you look after the transferred fat in the first weeks, including not sitting directly on it and avoiding nicotine, affects how much survives and how smooth the result is. The buttocks also look larger at first because of swelling; our article on BBL recovery and long-term results covers the timeline.

Judging natural results in photos#

Before-and-after photos help you see what a proportionate result looks like, as long as you read them carefully. Look for the same pose, distance and lighting in both photos, a side view as well as a back view, and photos taken several months after surgery rather than in the first weeks. Each photo shows one patient, whose result depends on their anatomy, fat and healing.

You can browse the BBL before and after gallery, and our guide on how to read BBL before-and-after photos explains what to look for.

Talking to your surgeon about the shape you want#

A clear conversation at the assessment makes a natural result more likely.

  • Bring photos of bodies with a build similar to yours, and say what you like about them.
  • Describe what bothers you now, for example flat upper buttocks, hip dips or a wide waist.
  • Ask how much volume is realistic for your frame and donor fat.
  • Ask where fat will be taken from and where it will be placed.
  • Ask what happens if more fat is lost than expected and whether a second session would be considered.

PSTG coordinates the assessment and the practical side of the trip, while the surgeon decides the shape that is safe and realistic for you. If you would like to know which shape could suit your body, you can contact us for a free, no-obligation assessment whenever you are ready.

Frequently Asked Questions

What is a BBL?

A Brazilian butt lift (BBL) is an operation that removes fat by liposuction from areas such as the abdomen, flanks or lower back and transfers part of it to the buttocks. It adds volume and shape to the buttocks while slimming the donor areas. It does not tighten loose skin.

What makes a natural BBL different from a dramatic one?

A natural BBL focuses on proportion, smooth transitions and a projection that suits your frame, rather than on maximum volume. A dramatic result adds more volume, which needs plenty of donor fat and is limited by the amount that can safely be transferred in one session. The surgeon advises what is realistic for your body.

Which BBL shapes can I ask for?

Patients often describe a soft hourglass, an athletic curve, hip dip correction, a slim-thick look or high projection. Which one is realistic depends on your donor fat, your pelvis and waist, and your skin. Hip dips can usually be softened rather than erased, because they partly reflect bone structure.

How much volume can be added in one session?

There is no fixed amount. It depends on how much usable donor fat you have, how much your skin and tissue can accept without excessive pressure, and the shape you want. Part of the fat is reabsorbed, and a second session is sometimes discussed later rather than overfilling in one operation.

How do fat transfer and implants compare?

Fat transfer uses your own tissue, slims the donor areas and tends to feel soft, but it needs enough donor fat and part of the fat is reabsorbed; its most serious risk is fat embolism. Implants can add volume for very slim patients and keep a fixed shape, but they carry risks such as infection, wound problems and displacement. Each has real risks, and the surgeon advises which suits your body.

Will people be able to tell I had a BBL?

A proportionate result usually reads as a more balanced figure rather than as surgery, but how noticeable it is depends on the volume and your starting shape. In the first weeks swelling makes the buttocks look larger than they will finally be. Small scars from the incisions fade over months but do not disappear completely.
